Chelsea have reportedly made a fresh offer to re-sign former midfield star Tiago Mendes and have put a two-year contract on the table.

The transfer is said to have been on the cards for some time, and it now appears as though it is close to completion as Diário de Notícias journalist Gonçalo Lopes believes that he will sign a deal to return to west London at some point this week.

Tiago was out of contract at Atletico Madrid having been a key part of the squad that won La Liga and reached the Champions League final last season.
But he now looks set for an amazing career swan song with the Blues as manager Jose Mourinho looks to add older players that understand the demands of competing in the Premier League and the Champions League simultaneously.
The 33-year-old first joined Chelsea a decade ago, but failed to become a regular at the flu and left to join French giants Lyon in 2004/05.
He since went on to feature for Juventus before linking up with Atletico in 2010.
