The former Manchester City striker has also been on the radar of Arsenal as the summer transfer window approaches, but the Reds are keen to bring him on board as a replacement for Luis Suarez just in case he does made an exit in the coming weeks.
The Italian international is said to be unhappy with life at the San Siro and is considering a return to England following several reported bust ups with coach Clarence Seedorf.
Balotelli spent three years at the Etihad Stadium between 2010-13, and scored 30 goals in 80 appearances, picking up a Premier League title and an FA Cup victory in the process.
Suarez has been in red hot form himself this season, and rumours in Europe are claiming that Real are preparing a world record £100m bid for the Uruguayan after the World Cup.
Arsenal are also said to be monitoring Madrid striker Karim Benzema and should they allow him to move on they will need a world class replacement to help build on their European success this term.
