Kramaric has been in excellent form this season, scoring 28 goals in all competitions and attracting interest from the Premier League as a result of his performances.
The 23-year-old will sign a four-and-a-half year deal with the West Londoners after completing a medical at Stamford Bridge and will join three other players from the club at Vitesse.
Currently Josh McEachran, Bertrand Traore and Wallace are with the Eredivisie club from the Bridge and Kramaric is likely to spend six months there before Jose Mourinho considers the forward for his first team squad.
Mourinho is looking for a long-term replacement for veteran Didier Drogba, who is likely to retire from football at the end of this season and start his venture into coaching.
Kramaric has been capped at international level after his impressive performances for Rijeka and to date has two goals in four caps for the Croatian national team.
